2. One (1) passport photo with a white background. Photograph quality requirements
3. Copy of Green Card or I-94 or Work Permit (except US National).
4. A completed and signed Visa Application form. (For foreign nationals of Eritrean origin, please fill out this Application Form)
5. Prepaid return UPS or FedEx label (address should be from sender to him/herself).
